Fibrocartilaginous Embolism

๐–๐ก๐š๐ญ ๐ข๐ฌ ๐ข๐ญ?
๐…๐ข๐›๐ซ๐จ๐œ๐š๐ซ๐ญ๐ข๐ฅ๐š๐ ๐ข๐ง๐จ๐ฎ๐ฌ ๐ž๐ฆ๐›๐จ๐ฅ๐ข๐ฌ๐ฆ is when a piece of ๐Ÿ๐ข๐›๐ซ๐จ๐œ๐š๐ซ๐ญ๐ข๐ฅ๐š๐ ๐ž (the main component of the discs between our spinal vertebrae) breaks off and enters the blood stream. From there, it forms an ๐ž๐ฆ๐›๐จ๐ฅ๐ฎ๐ฌ, or a foreign material in the blood, which can block blood flow.

๐–๐ก๐จ ๐ ๐ž๐ญ๐ฌ ๐ข๐ญ?
Technically any species can get this, but we most commonly think of it in large breed dogs. In particular, Miniature Schnauzers and Shetland Sheepdogs may be predisposed to this condition.

๐–๐ก๐š๐ญ ๐œ๐š๐ฎ๐ฌ๐ž๐ฌ ๐ข๐ญ?
There isnโ€™t a specific cause of FCE, however it has been associated with exercise or trauma. It is generally considered to be a ๐ฌ๐ฉ๐จ๐ซ๐š๐๐ข๐œ (randomly occurring) event.

๐–๐ก๐ฒ ๐ข๐ฌ ๐ญ๐ก๐ข๐ฌ ๐š ๐ฉ๐ซ๐จ๐›๐ฅ๐ž๐ฆ?
The major problem with FCE is that the vessels the embolus will obstruct are typically those that supply the spinal cord. This can lead to ๐ข๐ง๐Ÿ๐š๐ซ๐œ๐ญ๐ข๐จ๐ง (death of tissue due to inadequate blood supply) of the spinal cord. As you can imagine, this can cause a variety of neurologic symptoms in the poor dog! These signs can include weakness, paralysis, irregular gait and lack of pain perception.

๐‡๐จ๐ฐ ๐ข๐ฌ ๐ข๐ญ ๐๐ข๐š๐ ๐ง๐จ๐ฌ๐ž๐?
The veterinarian will initially complete a ๐ง๐ž๐ฎ๐ซ๐จ๐ฅ๐จ๐ ๐ข๐œ ๐ž๐ฑ๐š๐ฆ to assess the animalโ€™s nervous system. There are so many things that can cause neurologic signs in dogs, so it is important to be thorough! In particular for this condition, dogs will have asymmetric signs, and most commonly the hind limbs are affected. The clinical signs also have an ๐š๐œ๐ฎ๐ญ๐ž (sudden) onset, and the signs do not progress or get worse after about 12 hours. Based on these clues, the veterinarian may request ๐ฆ๐š๐ ๐ง๐ž๐ญ๐ข๐œ ๐ซ๐ž๐ฌ๐จ๐ง๐š๐ง๐œ๐ž ๐ข๐ฆ๐š๐ ๐ข๐ง๐  (MRI) in order to directly visualize the embolus. This will confirm the diagnosis!

๐‡๐จ๐ฐ ๐ข๐ฌ ๐ข๐ญ ๐ญ๐ซ๐ž๐š๐ญ๐ž๐?
Unfortunately there isnโ€™t a specific treatment for FCE, so these patients are often allowed to rest with supportive care. This allows time for the embolus to become ๐ซ๐ž๐œ๐š๐ง๐š๐ฅ๐ข๐ณ๐ž๐ (have a hole drilled through it) by the body and restore blood flow. These patients often require extensive rehabilitation after their initial rest period in order to restore full use of the limb. This recovery process can take weeks to months, but does have a fairly good prognosis!
