Theiler’s Disease

๐–๐ก๐š๐ญ ๐ข๐ฌ ๐ข๐ญ?
๐“๐ก๐ž๐ข๐ฅ๐ž๐ซโ€™๐ฌ ๐๐ข๐ฌ๐ž๐š๐ฌ๐ž is a mysterious disease that causes ๐š๐œ๐ฎ๐ญ๐ž ๐ก๐ž๐ฉ๐š๐ญ๐ข๐ญ๐ข๐ฌ (rapid onset liver inflammation) in horses. Ultimately, this disease results in severe liver failure.

๐–๐ก๐จ ๐ ๐ž๐ญ๐ฌ ๐ข๐ญ?
We see this disease in horses who have been administered an ๐ž๐ช๐ฎ๐ข๐ง๐ž-๐จ๐ซ๐ข๐ ๐ข๐ง ๐›๐ข๐จ๐ฅ๐จ๐ ๐ข๐œ. These are medical products that are created using blood or serum from another horse. The most commonly used biologic is ๐ญ๐ž๐ญ๐š๐ง๐ฎ๐ฌ ๐š๐ง๐ญ๐ข๐ญ๐จ๐ฑ๐ข๐ง, which is created using the serum of horses vaccinated against tetanus.

๐–๐ก๐š๐ญ ๐œ๐š๐ฎ๐ฌ๐ž๐ฌ ๐ข๐ญ?
We donโ€™t actually know the exact reason these biologic products cause Theilerโ€™s disease. There is some suspicion that it may be caused by a virus, currently called ๐“๐ก๐ž๐ข๐ฅ๐ž๐ซ ๐๐ข๐ฌ๐ž๐š๐ฌ๐ž-๐š๐ฌ๐ฌ๐จ๐œ๐ข๐š๐ญ๐ž๐ ๐ฏ๐ข๐ซ๐ฎ๐ฌ virus (a very creative name). It is thought that this virus may be related to human hepatitis virus.

๐–๐ก๐ฒ ๐ข๐ฌ ๐ญ๐ก๐ข๐ฌ ๐š ๐ฉ๐ซ๐จ๐›๐ฅ๐ž๐ฆ?
Theilerโ€™s disease causes massive ๐ก๐ž๐ฉ๐š๐ญ๐ข๐œ ๐ง๐ž๐œ๐ซ๐จ๐ฌ๐ข๐ฌ, or death of the liver cells. Horses will develop ๐ข๐œ๐ญ๐ž๐ซ๐ฎ๐ฌ (yellowing of the tissues) and severe ๐ง๐ž๐ฎ๐ซ๐จ๐ฅ๐จ๐ ๐ข๐œ ๐ฌ๐ข๐ ๐ง๐ฌ like maniacal behaviour, blindness and irregular, wobbly gaits. These neurologic problems occur because one of the liverโ€™s main jobs is removing ๐š๐ฆ๐ฆ๐จ๐ง๐ข๐š from the bloodstream. When liver failure occurs, ammonia accumulates in the blood, altering normal brain function.

๐‡๐จ๐ฐ ๐ข๐ฌ ๐ข๐ญ ๐๐ข๐š๐ ๐ง๐จ๐ฌ๐ž๐?
Blood work and ultrasound can be used to identify hepatic necrosis, however the history of receiving an equine-origin biologic is probably most important in making the diagnosis of Theilerโ€™s disease versus other causes of liver failure.

๐‡๐จ๐ฐ ๐ข๐ฌ ๐ข๐ญ ๐ญ๐ซ๐ž๐š๐ญ๐ž๐?
Unfortunately there is no specific treatment for this disease. Most horses recover with appropriate supportive care. In severe cases, the horse is often euthanized due to an extremely poor prognosis.

1-3) If you say the words โ€œTheilerโ€™s diseaseโ€ to any pathologist, they will probably reply with โ€œ๐๐ข๐ฌ๐ก ๐ซ๐š๐  ๐ฅ๐ข๐ฏ๐ž๐ซโ€. Thatโ€™s because the degree of hepatic necrosis is so severe with this disease that the liver turns into a floppy dish rag! Here you can see examples of floppy livers. 4) What Theilerโ€™s disease looks like under the microscope. Normally the liver is full of lovely ๐ก๐ž๐ฉ๐š๐ญ๐จ๐œ๐ฒ๐ญ๐ž๐ฌ (liver cells) forming a network. In Theilerโ€™s disease, you simply see liver goo.
