Nutmeg Liver

๐–๐ก๐š๐ญ ๐ข๐ฌ ๐ข๐ญ?
๐๐ฎ๐ญ๐ฆ๐ž๐  ๐ฅ๐ข๐ฏ๐ž๐ซ refers to a specific pattern we can see in the liver at necropsy, where the liver takes on the appearance of a nutmegโ€™s cut surface. This is a very characteristic sign of ๐œ๐ก๐ซ๐จ๐ง๐ข๐œ ๐ฉ๐š๐ฌ๐ฌ๐ข๐ฏ๐ž ๐œ๐จ๐ง๐ ๐ž๐ฌ๐ญ๐ข๐จ๐ง, however does not tell you much about the actual cause of the congestion.

๐–๐ก๐š๐ญ ๐œ๐š๐ฎ๐ฌ๐ž๐ฌ ๐ข๐ญ?
The basic underlying problem leading to passive congestion is increased pressure in the ๐ก๐ž๐ฉ๐š๐ญ๐ข๐œ ๐ฏ๐ž๐ข๐ง๐ฌ, which go from the liver to the circulation. This causes the blood in the liver to become ๐œ๐จ๐ง๐ ๐ž๐ฌ๐ญ๐ž๐ (not flowing) because it simply canโ€™t push against the pressure in its normal outflow vessel. This is almost always caused by some kind of heart issue, specifically issues affecting the right side of the heart, as it regulates the pressure within veins.

At the microscopic level, the blood in the liver tends to accumulate around the ๐œ๐ž๐ง๐ญ๐ซ๐ข๐ฅ๐จ๐›๐ฎ๐ฅ๐š๐ซ ๐ณ๐จ๐ง๐ž๐ฌ (around the hepatic veins), making these areas strikingly red. Elsewhere in the liver, the ๐ก๐ž๐ฉ๐š๐ญ๐จ๐œ๐ฒ๐ญ๐ž๐ฌ (cells of the liver) become swollen and take on a lot of fat, making these areas pale tan. This colour contrast produces the characteristic pattern we see at necropsy. Neat!
